Position Summary The Primary Care Provider (Family Med or Internal Med Physician) delivers preventive, chronic disease maintenance and episodic health care to patients of our Health Center within the identified scope of primary care practice.  The Provider supports the mission and goals of our Health Center. Utilizes professional skills to diagnose and treat patients who present with both acute and chronic health problems. Ensures efficient and effective patient-centered, team-based care through collaborative teamwork, problem solving, and communication.  Provides care consistent with medical best practices and the policies and procedures of the organization.
